Please enclose the chosen form of payment with the sample shipment. Prices are listed on the test description pages.

Checks:
Checks should be made out in the full amount specified on the test description page and payable to: Trustees, University of Pennsylvania/Dr. Giger

Credit Card:
Print and fill out the Credit Card Payment Sheet (PDF). Include the sheet with the sample submission form in the sample shipment. Your card will be billed for proper amount.
