Sending samples from animals from outside of the United States of America may require special shipping instructions and forms (attached outside of package). An example of a Proforma Invoice and declaration of animal samples is enclosed. Please check with your carrier as to what forms might be needed. Some carrier websites include Federal Express (http://www.fedex.com/us/), United Parcel Service (http://www.ups.com/), and the United States Postal Service (http://www.usps.gov/). An example of the required form can be found at http://www.usps.com/forms/_pdf/ps4000.pdf. In the past, Federal Express has required a "Proforma Invoice" written by the sender and a declaration of samples signed by a veterinarian.
