By filling the forms in *online*, you enable the lab to prepare to receive your samples before they arrive.
This dramatically improves the processing time for all tests.
Select the type of test required from the five sample submission forms.
Please complete the form with as much information as possible.
This helps us to process your samples and report the results back in a shorter time frame.
When the form is complete, click on the Submit Form button on the bottom of the page.
This will transmit the your data to PennGen.
The screen should now display all your order information, but the Submit Form button will have disappeared.
If it is still on the screen, then your request has NOT been submitted.
Print a copy of this form on your computer to submit with your samples.
*It is VERY important to send a printed copy of the order form with your samples!
*To print a copy either press Ctrl + P, or click on File and then Print.
For payment please include a check for the amount due made payable to "University of Pennsylvania Trustees/Dr. Giger."
Please include your tracking number on the front of the check.
For Credit cards, please print, fill out and mail in the Credit Card payment form.
Package the samples as required depending on the test, and mail to:

Packing Check List:
Ensure that all samples are marked clearly on either the blood tube itself, for EDTA blood submissions, or the cheek swab sleeve and Ziploc bag or envelope, for cheek swab submissions, with:
In the case of blood sample submission, package the blood so that tubes will not break. They should be sent by 2-3 day delivery and do not need to be refrigerated. Cheek swabs can be sent via regular post.
Enclose a check for $75 per animal, made out to the Trustees of the University of Pennsylvania/Dr Giger.
Enclose completed submission forms for each animal being tested.
Results will be mailed back to you as soon as the tests are complete.
In addition test results can be reported via email to the email address you have provided on the form.
For this please email penngen@vet.upenn.edu. Remember to include in your email your name, the date samples sent, the animal's call name, and the test that was requested.
